Completing your Eleos Progress Note in Streamline SmartCare

Once you've completed your session using Eleos either with MS Teams or Sensi device, sign into Streamline SmartCare and open a Progress Note.

Before starting to use Eleos, always make sure you are signed into the Eleos Extension.

In the Service Tab

1. Update Status to Show

2. Ensure the Time In roughly matches (+/- 10 minutes) the time you started the Teams meeting or when you clicked on the Sensi device

In the Notes Tab

1. Open Note tab; Eleos overlays on the narrative sections (Interventions/Response, Progress, Plan)

2. Expand to full screen to get a better view

   1. Key Moments and Interventions from the session appear in the Intervention/Progress section of the note

   2. Assessments and Progress from the session appear in the Describe the client’s response to the intervention, progress made toward goals and client strengths… section of the note

   3. The Plan from the session appears in the Document the plan section of the note

3. Review the various Eleos suggestions and complete your Progress Note.

4. Once complete, save and sign as your normal workflow.
